Dexamethasone injection should be prescribed pre-emptively, where required at the end of life, usually following the recommendation of the specialist palliative care team.

When given by injection, usually subcutaneously, dexamethasone injection is expressed in terms of dexamethasone (base):

Dexamethasone (base) 3.3mg/1ml injection = dexamethasone phosphate 4mg (oral equivalent)
Dexamethasone (base) 6.6mg/2ml = dexamethasone phosphate 8mg (oral equivalent)

Liquid Hydrocortisone is initiated by a specialist but is suitable for primary care to continue and take over ongoing prescribing responsibility. The specialist to provide the primary care clinician with the necessary information and support in order for treatment to be managed safely in primary care. Referring to guidance within the Efmody SCP, hydrocortisone immediate release is referenced within the document as the first line.
