Antiepileptic drugs: updated advice on switching between different manufacturers’ products Nov 2017

The risk of serious skin-related adverse drug reactions, including Stevens-Johnson syndrome, occurring with carbamazepine may be increased in the presence of the HLA-A*3101 allele in patients of European descent or Japanese origin. However, there are currently insufficient data to support screening for this allele before starting carbamazepine treatment. Patients of European descent or Japanese origin who are known to be positive for this allele should only receive carbamazepine, oxcarbazepine or eslicarbazepine after careful consideration of the benefits and risks.

Update section 4.4 has been updated to highlight that pregabalin should not be used during pregnancy unless the benefit to the mother clearly outweighs the potential risk to the foetus.

*pregabalin has been associated with reports of respiratory depression, in some cases without concomitant opioid treatment
*consider whether adjustments in dose or dosing regimen are necessary for patients at higher risk of respiratory depression, this includes people: *with compromised respiratory function, respiratory or neurological disease, or renal impairment
*taking other CNS depressants (including opioid-containing medicines)
*aged older than 65 years

SPC update - sections 4.4 and 4.5 advise caution when used with opioids due to risk of CNS depression - this increased risk was observed at low doses of pregabalin (≤ 300 mg) and there was a trend for a greater risk at high doses of pregabalin.

Beginning of safety review triggered by a study reporting an increased risk of neurodevelopmental disabilities in children with prenatal exposure following an observational study reporting an increased risk of neurodevelopmental disabilities in children whose mothers took topiramate during pregnancy.
Topiramate is already known to be associated with an increased risk of congenital malformations
and effects on foetal growth if used during pregnancy.
Actions
Continue to counsel patients who can become pregnant on the known and emerging risks of topiramate for an unborn baby and on the need to use effective contraception throughout use.

Subcutaneous Levetiracetam in Palliative and End of Life Care (Adults)

Use of off-label subcutaneous levetiracetam offers the possibility of maintaining seizure control when the oral route is lost without increasing the level of sedation. The use of medicinal products off-label is widespread. Surveys say that up to one quarter of all prescriptions in palliative care come into this category. This includes the use of two or more medications in a syringe driver. The British Pain Society and Association for Palliative Medicine have produced recommendations for
this practice. It is recommended to initiate SC levetiracetam only following advice from the Palliative Care Team or Hospice.
